Overview

Community Health Partners (CHP) is seeking a fulltime Nurse Practitioner (NP) or Physician Assistant (PA) to join our wellestablished outpatient endocrinology practice in Fresno CA. This is a unique opportunity to work alongside a collaborative team of four Endocrinologists and two experienced Advanced Practice Providers (APPs) in a thriving and comprehensive center dedicated to providing exceptional endocrine care.


Practice Highlights:
Stateoftheart resources including inhouse ultrasound and bone density services
Onsite licensed reference clinical laboratory
Access to an Endocrine Nurse for infusions and provocative testing
Collaborative care with dieticians and expertise in glucose sensor technology
Integrated tools for meter pump and sensor downloads at each visit

Responsibilities

The Community Health System is a locally owned notforprofit healthcare network based in Fresno California. Community is the regions largest healthcare provider and private employer. We operate a physician residency program with one of the nations top medical schools the University of California San Francisco. Community is home to the only Level I Trauma Center and comprehensive burn center between Los Angeles and Sacramento. We also serve as the areas safety net provider. Accountabilities

1. Provides direct assessment evaluation and primary patient care complying with medical protocols and procedures.
2. Provides direct patient management in collaboration with physicians; participates in daily rounds patient assessments and care plan development.
3. Orders interprets and evaluates diagnostic tests to identify and assess patients healthcare needs. Furnishes medications based upon written protocols.
4. Ensures that patients are counseled regarding treatment plans and the use dosage and possible side effects of medications.
5. Makes referrals to appropriate providers when patients require subspecialty medical attention beyond the areas scope of service.
6. Obtains prior authorizations from insurance companies.
7. Documents and maintains precise and complete records of patients.
8. Distinguishes between normal and abnormal findings.
9. Provides education training information and consultation services to interdisciplinary team patients family and caregivers.
10. Participates in quality assurance activities including assistance in development and implementation of new policies and procedures recommends updates to current policy as needed.
11. Performs other duties as assigned.

Qualifications

Education
Completion of a Nurse Practitioner or Physician Assistant program required
Credentialed by the CMC Medical Staff Office as an Allied Health Professional and privileged to perform Nurse Practitioner or Physician Assistant Standardized Procedures required

Experience
2 years of clinical experience in the area of specialty preferred

Licenses and Certifications
If qualifying as a Nurse Practitioner the following is required
o NP Current State of California Nurse Practitioner License required
o NPF Nurse Practitioner Furnishing Number required
o If qualifying as a Physician Assistant the following is required
o PA State of California Physician Assistant License required
o DEA DEA Controlled Substance Number required
o BLS Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
o Ability to participate in a federally funded program (Medicare and Medicaid) required
o NPC National Certified Nurse Practitioner by ANCC (American Nurses Credentialing Center) or AANP (American Academy of Nurse Practitioners) preferred
o CRNFA Certified RN First Assistant preferred
