Job Description

Overview

The Hospice LPN provides skilled nursing care to patients in their homes under the supervision of a Registered Nurse. They provide service in accordance with the Home Health Agency (HHA) philosophy standards policies and scope of practice.

Responsibilities

  • Provides skilled nursing in the home as ordered on the plan of treatment and nursing care plan.
  • Evaluates the significance of assessment findings and communicates any pertinent information about the patients clinical status and ongoing needs to others in a timely fashion
  • Assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
  • Demonstrates knowledge of medications and their correct administration based on age of the patient and patients clinical position.
  • Follows the seven 7 medication rights and reduces the potential for medication errors.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Functions as a team member demonstrating collaboration with and responsibility to patient physician interdisciplinary team.
  • Adheres to all HHA payroll billing and documentation policies and procedures.
  • Participates in HHAs other mandated activities as requested by Clinical Supervisor or Patient Care Services Director including inservices and Upper Respiratory (UR)/ Perfusion Index (PI) committee.
  • Performs patient care responsibilities considering needs to the standard of care for patients age.
  • Articulates HHA grievance/concerns procedure.
  • Articulates HHA adverse event/incident reporting procedure.
  • Articulates organizations emergency management plan.
  • Assists the physician and registered nurse to perform specialized procedures.
  • Works as part of the interdisciplinary team.
  • Completes patient documentation on time.
  • Attends inservices and staff meetings as needed.
  • Completes assigned training on time.
  • Completes annual education requirements.
  • Maintains patient confidentiality at all times.
  • Performs other duties as assigned.

Qualifications

  • Graduate of an accredited nursing program.
  • Current state licensure as a LPN.
  • Experience as an LPN commensurate with one of the following:
  • One 1 year experience in an acute care setting within the last 24 months;
  • Six 6 months home care experience within the last 12 months.
  • Prior home healthcare experience is preferred.
  • Valid state drivers license and reliable automobile current automobile insurance and being willing to operate personal car(s) necessitated by nature of job.
  • Current TB testing.
  • Current CPR card.
  • No findings relating to client abuse neglect or misappropriation of client property.
