Job Description
Overview
The Hospice LPN provides skilled nursing care to patients in their homes under the supervision of a Registered Nurse. They provide service in accordance with the Home Health Agency (HHA) philosophy standards policies and scope of practice.
Responsibilities
- Provides skilled nursing in the home as ordered on the plan of treatment and nursing care plan.
- Evaluates the significance of assessment findings and communicates any pertinent information about the patients clinical status and ongoing needs to others in a timely fashion
- Assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
- Demonstrates knowledge of medications and their correct administration based on age of the patient and patients clinical position.
- Follows the seven 7 medication rights and reduces the potential for medication errors.
- Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
- Functions as a team member demonstrating collaboration with and responsibility to patient physician interdisciplinary team.
- Adheres to all HHA payroll billing and documentation policies and procedures.
- Participates in HHAs other mandated activities as requested by Clinical Supervisor or Patient Care Services Director including inservices and Upper Respiratory (UR)/ Perfusion Index (PI) committee.
- Performs patient care responsibilities considering needs to the standard of care for patients age.
- Articulates HHA grievance/concerns procedure.
- Articulates HHA adverse event/incident reporting procedure.
- Articulates organizations emergency management plan.
- Assists the physician and registered nurse to perform specialized procedures.
- Works as part of the interdisciplinary team.
- Completes patient documentation on time.
- Attends inservices and staff meetings as needed.
- Completes assigned training on time.
- Completes annual education requirements.
- Maintains patient confidentiality at all times.
- Performs other duties as assigned.
Qualifications
- Graduate of an accredited nursing program.
- Current state licensure as a LPN.
- Experience as an LPN commensurate with one of the following:
- One 1 year experience in an acute care setting within the last 24 months;
- Six 6 months home care experience within the last 12 months.
- Prior home healthcare experience is preferred.
- Valid state drivers license and reliable automobile current automobile insurance and being willing to operate personal car(s) necessitated by nature of job.
- Current TB testing.
- Current CPR card.
- No findings relating to client abuse neglect or misappropriation of client property.
