POSITION DESCRIPTION Child and Family Services Foster Care Job Title: Foster Care Case Manager Reports To: Foster Care Supervisor Status: FullTime FLSA Status: Exempt Our Mission: To provide expert comprehensive services in southeastern Michigan that strengthen children adults and families impacted by abuse and neglect autism developmental disabilities and mental health challenges so they are successful in their communities. Job Summary: The Foster Care Case Manager provides case management services to children in foster care and their families and assists them with reaching timely permanency. Primary Duties and Responsibilities: - Develops and implements a permanency plan for each assigned child.
- Develops appropriate treatment plan and goals for all consumers served.
- Provides necessary resources and referrals to meet goals.
- Participates in required trainings scheduled team meetings and court hearings.
- Completes facetoface contacts as required.
- Assists with emergency situations by covering shift(s) as necessary.
- Performs other duties as assigned.
Job Qualifications: - Must be able to work a flexible work schedule that allows for support to consumers during nontypical business hours.
- Proficient in Microsoft Word Outlook and Excel.
- Concise grammatical skills.
- Excellent oral and written communication skills.
- Ability to provide culturally competent services that shows sensitivity to the service populations cultural and socioeconomic characteristics.

Education Certificates Licenses Registrations: - Bachelors degree in a social work or related human services field.
- Must be able to attend and complete 9week foster care preservice training through the Department of Health and Human Services (DHHS). Completion is achieving a passing score.
- Valid Michigan Drivers License appropriate insurance and use of own vehicle.
Working Conditions: - Works in an office environment and in the community as needed.
- Nontraditional hours occur often due to the need for evening and weekend visitation for children and birth parents as well as accommodation of foster parents schedules.
