Job Title: Remote RN Discharge Planner (TemptoPerm)
Pay Rate: $40$42/hr
Schedule: Monday Friday 8:00 AM 5:00 PM PST
Location: Remote but candidate must reside in California and hold a valid CA RN license
Job Description:
A Managed Care Organization is seeking experienced Registered Nurses to assist with discharge planning for Health Plan members. The RNs will work closely with California hospitals skilled nursing facilities (SNFs) and other lower levels of care to ensure members transition safely and appropriately. This role requires strong clinical expertise and an indepth understanding of MediCal postacute care benefits including criteria for coverage and contractual agreements related to postacute services. The position is remote with an exception allowing candidates to reside and be licensed in California. This is a temptohire opportunity.
Key Responsibilities:
Collaborate with hospitals SNFs and other lowerlevel care entities to coordinate member discharges.
Utilize MCG criteria to assess and determine appropriate discharge plans.
Review and interpret facility contracts related to postacute levels of care.
Ensure members transition efficiently and effectively while adhering to MediCal postacute care benefits.
Communicate compassionately with members and providers to facilitate seamless care transitions.
Demonstrate a high level of accountability in ensuring timely and appropriate discharge planning.
Required Skills & Qualifications:
Active CA Registered Nurse (RN) license.
Reside in California.
Minimum of 5 years of clinical experience in acute care settings such as medsurg/telemetry ICU or ER.
Minimum of 2 years of hospitalbased discharge planning experience with expertise in transitioning patients to home SNF custodial care LTAC and ARU.
Strong understanding of MediCal postacute care benefits and covered services.
Experience with MCG inpatient criteria.
Familiarity with facility contract language for postacute levels of care.
Ability to manage multiple priorities and work effectively in a remote environment.
Excellent communication and collaboration skills to engage with members providers and healthcare entities.
Preferred Experience:
Previous experience in managed care or health plan settings.
Familiarity with case management and care coordination.
Additional Information:
This is a temptoperm position based on performance and business needs.
Candidates must be available for fulltime hours and demonstrate strong accountability in discharge planning.
