The Role:
As a Psychiatric Nurse Practitioner at Senior Doc youll play a pivotal role in delivering compassionate individualized mental health care to seniors in the comfort of their homes assisted living facilities and nursing homes. Your responsibilities will include:
- Conducting comprehensive psychiatric assessments including evaluating mental health status cognitive function and behavioral concerns in elderly patients.
- Developing and implementing personalized mental health care plans addressing conditions such as depression anxiety dementia and other geriatric psychiatric disorders.
- Prescribing and managing psychotropic medications while monitoring patients for side effects interactions and treatment efficacy.
- Collaborating seamlessly with interdisciplinary teams including primary care physicians therapists and social workers to ensure holistic and integrated patient care.
- Providing crisis intervention and mental health education for patients and their families enhancing their understanding and management of mental health conditions.
- Building trusting relationships with patients families and caregivers through empathy active listening and exceptional communication.
- Documenting and monitoring progress to ensure the ongoing effectiveness of treatment plans adjusting care strategies as needed.
