This is a remote position.
Duties and Responsibilities

Conduct Virtual Assessments and Consultations
Perform video or phone-based evaluations reviewing patients medical histories medication regimens and current health status.

Provide Patient Counseling and Education
Deliver guidance on preventive care chronic disease management lifestyle modifications and medication adherence.

Coordinate Care and Make Referrals
Work with health center care teams specialists and community-based providers to ensure seamless coordination of care including necessary referrals and follow-ups.

Maintain Documentation
Accurately document patient encounters and care plans in the electronic medical record system in a timely and compliant manner.
Requirements
Experience and Qualifications

Active Nurse Practitioner license in multiple U.S. states

Proficiency with EMR systems and virtual care platforms

Background in primary care chronic care and medication reviews

Strong communication skills and ability to work both independently and within a collaborative care team

Flexibility to occasionally provide care outside standard business hours
Qualifications: High school diploma or equivalent required; some college coursework in healthcare or administration preferred Strong attention to detail and excellent organizational skills Basic knowledge of EMRs or willingness to learn Ability to manage time effectively and meet deadlines Proficient in Microsoft Office Suite (especially Word and Excel) Strong communication skills, both written and verbal Ability to handle confidential information with professionalism and discretion
