Peer Support Specialist Permanent Supportive Housing
Cascade AIDS Project (CAP) is the oldest and largest AIDS Service Organization (ASO) in Oregon and Southwest Washington that provides the LGBTQ community and beyond with compassionate healthcare. CAPs mission is to promote wellbeing and advance equity by providing inclusive health and wellness services for LGBTQ people people affected by HIV and all those seeking compassionate care. This is a Full Time role and will be colocated at CAPs Davis Street office and up to 2 days a week at the Joyce Apartments where some CAP permanent supportive housing clients reside. Travel is required throughout the Portland Metro service area for meetings and client home visits. The schedule for this role is MondayFriday 9am5pm some nights and weekends may be required.
Starting pay is $22.77 an hour which is equivalent to just over $47370 a year. The Peer support specialists Pay increases with tenure and the top of the pay scale is 26.52. This is a union represented position so the compensation benefits and conditions of work are collectively bargained.
Who You Are
- If there is a history of substance abuse or addiction the Peer Support Specialist must have a minimum of two years of continuous and current sobriety.
- Knowledgeable about the lived experience of housing instability mental health issues or substance us issues and willingness to mindfully share that experience.
- Within 3 months of hire successful completion of an Oregon Health Authority (OHA) Addictions & Mental Health (AMH) approved Peer Support Specialist Training and certification as a Peer Support Specialist
- Basic computer and keyboard proficiency using Microsoft Office software (Word Excel Outlook) and working knowledge of the internet
- Good organizational and time management skills
- Ability to work independently with accountability; exercise sound judgment and discretion; maintain healthy boundaries respect and discretion at all times
What Youll Do
- Collaborate with medical case managers housing case managers service navigators medical providers or other providers involved in client care to provide coordinated comprehensive care to people who are transgender or nonbinary including those who live with HIV/AIDS focusing on fulfilling their goals.
- Make appropriate referrals to mental health and substance abuse resources psychosocial support or other community partners and assist clients in accessing these services.
- Accompany clients to appointments to facilitate access to medical care substance use treatment mental health services housing and other services.
- Provide housing sustainability support which may include eviction prevention support advocacy with landlords with and on behalf of clients support understanding and complying with rental agreements and general information and referral assistance.
- Keep accurate complete timely and current written and database records/files including timely data entry.
- Meet all contract requirements and provide supporting documentation including program performance data and reports as required.
