Responsibilities and Duties:
- Evaluate treat and direct skilled treatment for patients for whom physical therapy is medically necessary. Document this need clearly.
- Maintain ongoing documentation in which goals progress toward functional goals and medical necessity are clearly stated.
- Complete all required documentation (precertifications physician reports daily notes charge tickets etc. legibly appropriately and in a timely fashion to meet facility physician and payor deadlines.
- Maintain open and respectful communication with coworkers physicians patients family members and thirdparty payors at all times.
- Attend and/or participate in facility meetings as directed by the Center Manager (may include utilization review infection control disaster preparedness etc.
Requirements:
- Resume
- NM Physical Therapy License
- Current BLS/CPR (AHA)
- Skills checklist
- 2 current Supervisor references
- Physical (within the last 12 months)
- PPD (within the last 12 months)
- MMR Varicella Hep B or Titers Vaccination
- Tdap/Flu (or decline)
- Covid Fully Vaccinated
- Exams
- LGBT Training Certification
- Must be able to pass background/ urine drug screen
