About RAAC Rapid Access Addictions Clinic (St Pauls):
We area lowbarrier outpatient clinic providing consultation and stabilization for individuals with substance use disorder or reliance including alcohol stimulant and opiate use disorders. You can learn more about our clinic here.

About Crosstown Clinic:
Crosstown Clinic is a pioneering leader in substance use treatment recognized as the first clinic in North America to offer diacetylmorphine (prescription to individuals with severe substance dependence for whom conventional treatments have proven ineffective. The clinic focuses on delivering Injectable Opioid Agonist Therapy (iOAT) and provides lifechanging care to clients who require this specialized treatment approach. About 8A:
The Urban Health Acute Care Unit 8A) at St. Pauls Hospital is a 24 bed unit that provides acute hospital care for patients with infectious diseases addiction HIV/AIDS and chronic/acute illness associated with marginalization. The unit provides a muchneeded multidisciplinary service which is patientcentred traumainformed and includes a progressive harm reduction approach to addictions management. The care team is particularly experienced in the management of infections among marginalized patients who are often underhoused and with few community supports.
The role:
Reporting to the Patient Care Manager or Resident Care Manager and working in collaboration with the interdisciplinary team the Licensed Practical Nurse (LPN) is responsible for providing nursing care to an assigned group of patients/residents according to established policies and procedures and the Standards of Practice for the Licensed Practical Nurses of British Columbia. The LPN functions as an independent practitioner while recognizing situations such as increased patient acuity or care that falls outside the scope of an LPN as appropriate for accessing the RN/RPN for collaborative support and/or clinical intervention.
Your duties will include:
- Assists patients/residents in meeting physical social spiritual and psychological needs
- Consistent with mission vision and values of Providence Health Care (PHC)
- Implements established protocols
- Participates in care planning assessments interventions and evaluations
- Documents information and advocates for patients/residents
- Attends various meetings including care conferences
- Assists with orientation of new staff and students
- Responds to and reports incidents following established procedures
- Performs clerical duties as required
What You Bring
Education:
- Graduation from a recognized program for Practical Nurses or an equivalent combination of education training and experience.
- Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant.
- Current basic cardiac life support certification.
Skills & Abilities:
- Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service
- Physical ability to carry out the duties of the position
- Ability to apply decision making and analytical skills
- Ability to role model conflict resolution strategies
- Ability to prioritize and organize work
- Ability to teach and identify patient/resident learning needs

Your Day to Day
- Provide safe and respectful nursing care adhering to established policies individualized care plans and priorities.
- Conduct thorough assessments monitor signs/symptoms and communicate with the RN/RPN as needed.
- Prepare patients for procedures transfers and admissions coordinating with the interdisciplinary team.
- Administer medications per protocol monitor effects and maintain communication with the team.
- Utilize evidencebased resources to stay updated on nursing practices.
- Respond to incidents such as codes injuries spills and alarms following procedures.
- Assist with staff orientation demonstrating procedures and providing support.
- Address patient/resident and family learning needs provide educational materials and make referrals.
- Attend care planning meetings provide input and advocate for patients/residents.
- Collaborate with the team to develop and evaluate individualized care plans.
- Assist with discharge planning based on patient status and external supports.
