Paradigm Rehabilitation is a multidisciplinary practice offering Physical Therapy Occupational Therapy and Speech Therapy for children across the five boroughs of New York City Albany Schenectady Buffalo Rochester and Westchester. Were seeking dedicated professionals to join our team and provide highquality services in schools homes daycare centers and other settings. Physical Therapist Requirements:
- Graduated from an accredited Physical Therapy (PT) college or university.
- Unencumbered Physical Therapy (PT) license and registration for New York.
- 1 year of Home Health Outpatient experience preferred as a Physical Therapist
- Conduct home visit Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions functional capabilities limitations and potential for rehabilitation.
- Basic computer literacy skills
- Proficient in EMR documentation (we use Hello Note but experience in this system is not required).
- Excellent communication between the office staff and therapists
- Maintain uptodate CPR certification and adhere to all safety protocols
- Manage a minimum caseload of 45 patients
$50 $70 an hour
