Job Description
Job Title: Physical Therapist
Facility: Monadnock Community Hospital

Contract: Permanent
Shift: 3:30 PM 12:00 AM
Salary Range: $67000 $90000 (dependent upon experience)
Guaranteed Hours: 37.5 hours per week

The Physical Therapist provides patient/resident therapy evaluations and/or treatments according to established policies and procedures for pediatric to geriatric patients. This job description in no way states or implies that these are the only duties to be performed by this employee. He or she will be required to follow any other instructions and to perform any other duties requested by his or her supervisor.
Job Responsibilities:- Evaluate patient needs and create treatment plans with measurable goals.
- Provide and assess therapy treatments per department policies.
- Educate patients families and team members to support treatment goals.
- Deliver care tailored to the patients age physical ability and development.
- Maintain accurate and timely documentation.
- Communicate patient status progress and discharge plans with the care team.
- Supervise therapy assistants and delegate treatments appropriately.
- Conduct reevaluations complete daily billing and schedule patients efficiently.
- Engage in continuing education provide inservice training and mentor students.
- Participate in department projects safety procedures and hospital initiatives.
