Job Description
Job Title:Social Worker Older Persons Team
Location:Wolverhampton
Rate:Up to 33.20 per hour
Working Hours:Fulltime 37 hours per week) or parttime options available
Contract:Initial 3month contract with scope to extend
An exciting job role has recently become available in Wolverhampton. They are looking for a new member for their Older Persons Team. As an Adult Social Worker you will play a crucial role in supporting vulnerable adults within our community. Youll work closely with individuals families and other professionals to promote independence wellbeing and social inclusion. Your compassionate approach and strong assessment skills will contribute to positive outcomes for those you serve.
What will your responsibilities be
Conduct comprehensive assessments of individuals needs considering physical emotional and social factors. Develop personalized support plans that empower clients to achieve their goals. Collaborate with health professionals care providers and community resources to coordinate services. Identify and respond to safeguarding concerns promptly. Assess risks related to vulnerable adults and implement appropriate interventions. Work within legal frameworks to ensure the safety and wellbeing of service users. Provide emotional support counseling and advocacy. Assist clients in accessing relevant services benefits and housing. Facilitate group sessions and workshops to enhance social skills and resilience. Liaise with healthcare professionals housing officers police and other agencies. Attend case conferences and multidisciplinary meetings. Advocate for clients rights and needs within the wider system.

Qualifications and Experiences:
- Degree in Social Work or equivalent qualification.
- Registration with Social Work England.
- Experience working with adults in a social care setting.
- Knowledge of relevant legislation including the Care Act 2014 and Mental Capacity Act 2005.
