Position Overview: and nbsp;
We are seeking a dedicated MSW (Master of Social Work) to join our team and and nbsp;
provide Community Integration Counseling (CIC) Services. The MSW will work with and nbsp;
individuals directly to develop and implement personcentered plans that support and nbsp;
the individuals goals and preferences and promote independence and community and nbsp;
integration. and nbsp;
Responsibilities: and nbsp;
Conducting an initial assessment to understand the individuals strengths and nbsp;
needs and goals for community integration and nbsp;
Developing a personcentered plan that outlines the support needed to help and nbsp;
the individual engage more fully in their community and improve their overall and nbsp;
quality of life. Providing counseling services to help individuals manage mental health and nbsp;
conditions behavioral challenges or other issues that may hinder community and nbsp;
involvement. and nbsp;
Life skills training such as social skills communication skills and and nbsp;
problemsolving techniques to help individuals engage effectively in community and nbsp;
settings. and nbsp;
Connecting individuals with local community resources such as vocational and nbsp;
programs recreational activities educational opportunities or social groups. and nbsp;
Encouraging participation in community events and activities fostering and nbsp;
relationships and helping individuals navigate their environments in a way that and nbsp;
promotes independence. and nbsp;
Facilitating access to vocational training or supported employment opportunities and nbsp;
that help individuals enter or reenter the workforce. and nbsp;
Advocating on behalf of individuals to ensure they have access to the services and nbsp;
programs and support they need in the community. and nbsp;
Supporting family members or caregivers with information and resources to help and nbsp;
them provide effective care and encouragement. and nbsp;
Developing and implementing crisis intervention strategies when needed and nbsp;
ensuring that individuals have the support they need to stay safe and continue and nbsp;
progressing. and nbsp;
Referring clients to other specialized services as needed such as mental health and nbsp;
treatment addiction services or specialized rehabilitation. and nbsp;
Maintaining thorough records of client progress goals and service plans to and nbsp;
ensure compliance with New York State regulations.Qualifications: and nbsp;
Bachelors degree in social work psychology or a related field. and nbsp;
Two 2 years of experience providing adjustment related to counseling to and nbsp;
seniors and/or individuals with physical and/or cognitive disabilities and and nbsp;
their families to be considered qualifying experience. and nbsp;
Knowledge of the NHTD/TBI program and regulations. and nbsp;
Strong communication and interpersonal skills. and nbsp;
Ability to work independently and as part of a team. and nbsp;
Proficiency in computer skills including electronic health record and nbsp;
Systems. Valid drivers license and reliable transportation. and nbsp;
