The successful applicant will be part of a multidisciplinary team responsible for the care of patients and families within the Medicine programs and will report to the Clinical Services Manager with professional accountability to the professional leader for social work. The social worker will be an integral part of the social work professional team participating in yearly strategic planning initiatives and corporate professional development.

Qualifications :
- Master of Social Work
- BCLS required
- Current registration and in good standing with the Ontario College of Social Workers and Social Service Workers
- Minimum 3 years relevant experience in assessing and treating patients with a focus on Neurological Rehabilitation including MildModerate Traumatic Brain Injury (TBI) Concussion Stroke/Aphasia etc.
- Experience in conducting/facilitating psychoeducational groups required
- Experience working with individuals who have severe and persistent mental illness metabolic and concurrent disorders required
- Previous experience in the areas of return to work planning functional evaluation and work conditioning an asset
- Cognitive Behavioral Therapy (CBT) and/or certification in other psychotherapy modalities preferred
- Training in Supportive Communication for Aphasia (SCA) techniques an asset
- Excellent written communication skills required for comprehensive reports
- Experience providing services to third party payers including WSIB
- Ability to deliver counselling and psychotherapy within the legislative requirements
- Experience delivering psychotherapy to WSIB patient population addressing psychosocial barriers in order for safe return to work
- Strong counseling skills in individual family and group mediums
- Knowledgeable in the provision of professional social work services to clients and their families/significant others/care givers (such as individual group marital or family counseling discharge panning etc.
- Demonstrated effective communication team building problem solving and conflict resolution skills
- Demonstrated ability to engage and work collaboratively with all levels and members of the inter professional team
- Demonstrated effective judgment and innovative change management skills
- Demonstrated ability to utilize quality safety and riskbased frameworks to utilize critical thinking priority setting and problemsolving skills
- Excellent written verbal communication and documentation skills
- Detail oriented with excellent organization and time management skills

HOURS: Monday Friday Days/Evenings with variable start/end times (Subject to change in accordance with Operational Requirements which may include evenings and weekends based on program growth)
Salary:
Minimum: $44.69 per hour
Maximum: $57.93 per hour
