Job Title: Social Worker Case Manager
Facility: Swedish Edmonds Campus
Address:th Ave W Edmonds WA 98026
Contract: 13 weeks
Shift: 08:00 AM 4:30 PM 5x8
Pay Rate: $62/hr (W2
We are seeking a compassionate Social Worker Case Manager to join the team at Swedish Edmonds Campus. The ideal candidate will provide case management services support patients and families in navigating healthcare challenges and coordinate postdischarge resources to ensure continuity of care.
Responsibilities:- Assess patients social emotional and financial needs to develop individualized care plans.
- Provide crisis intervention emotional support and counseling to patients and families.
- Coordinate discharge planning referrals and community resource connections.
- Advocate for patients regarding healthcare access insurance and social services.
- Collaborate with medical teams to ensure comprehensive and holistic patient care.
- Maintain accurate documentation and comply with facility policies and regulatory requirements.
- Masters degree in Social Work (MSW) required.
- Active Social Worker license (LICSW LCISW or equivalent).
- Minimum 1 year of case management or hospitalbased social work experience.
- BLS certification (if required by facility).
- Knowledge of healthcare systems case management and discharge planning.
