Job Description
Summary: YRMC is in need of general cardiology coverage
Summary: Board Certified Infectious Disease Clinician needed for small outpatient clinic. This is a one provider practice. Seeing about 1014 outpatients per day and monitoring/ordering their treatments. We work closely with local infusion centers a wound care center and the Dignity Health Hospital.
Rate Caps for Submission:
Malpractice Must be included in rates.
Orientation half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
Hourly BR and below Per hour not OT
Holiday Must be 1.5x the hourly rate. Per holiday hour no OT
Schedule/Availability Requirements:
Minimum of 2 weeks on preferably 4 /Full Time site will not consider: 1 week per month or 1 week on 1 week off
Facility Location: 811 Ainsworth Dr Suite 106 Prescott AZ 86301
Setting: attached to hospital but outpatient clinic
Duties:
Days per month: MF
Required Procedures: in office dressing changes/wound care
EMR: CERNER
Support Staff: 1 MA 1 front office
Reason for Coverage: Long term provider is retiring
Dealbreakers: (need someone who can come for an extended time not just 13 weeks.)
Travel Preferences:
Preferred Lodging: Preferred hotels
Air Travel: Fly into Sky Harbor International in Phoenix and use rental car to drive 100 miles to Prescott.

Travel exception requests will be denied if this information is not part of the provider presentation. Credentialing with other facilities N/a only with Dignity Health YRMG for this one location
Billing enrollment Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less No

712 patients per day provider reviews all incoming referrals to approve
First day info orientation to the clinic space and EMR training on first day of patients Cerner support staff is in clinic to answer any questions
