Job Summary

Our client is a leading charity operating across Ireland supporting some of the most marginalised individuals couples and families experiencing homelessness.

They are currently seeking an experienced Community Integration Case Manager to join their Dublin operation.

This is a fixed-term contract to permanency working 35 hours per week.

    Key Responsibilities
  • In partnership with service users to assess their needs and develop workable support plans with regular reviews on the progression of agreed goals.
  • Life Skills Assessment and Practical Support - In collaboration with the PSS building on existing life skills interventions to empower individuals to live independently.
  • Promoting the social inclusion of Housing First clients into local communities through effective use of community development principles and practices.
  • To support and facilitate clients to access a range of opportunities in their local community
  • Connecting people to non-medical sources of support or resources in their community.
  • Utilise the HSE social prescribing program which addresses health influenced by social factors like poverty and isolation by connecting individuals to non-clinical community supports that enhance overall well-being.
  • To develop individualised plans around personal development accessing training employment and enjoyable/productive meaningful use of time based on the on unique attributes preferences and aspirations of the individual.
  • In partnership with service-users assess their post-settlement tenancy sustainment and social integration needs and develop & implement post-Settlement Support Plans.
  • Monitoring of support requirements of participants to sustain their accommodation i.e. rent payments etc. and work with the participant utilising motivational and recovery orientated techniques to ensure sustainment of accommodation.
  • Ensure support plan outcomes are focused on the development of a socially valued role.
  • Support with accessing local groups and activities for example sporting music arts and cultural activities green space or gardening debt advice or budgeting physical activity and leisure library membership volunteering housing maintenance and more
  • To establish and maintain positive trusting relationship with the person being supported that is based on respect equality and that promotes independence.
  • To ensure all person specific entitlements and opportunities are utilised to their maximum benefit.
  • To develop strategic links collaborations and robust pathways with relevant external stakeholders and agencies focusing on various levels of participation & sustainable social role development.
  • Develop and maintain positive working relationships with all stakeholders and develop productive positive working networks within the wider community
  • Lead on developing local forums and escalate issues as appropriate.
  • Ensure Depauls Good Neighbourhood Policy is being implemented within the service and support service users in adhering to policy expectancies
  • Delivering on DRHE Case and Case Management Protocols.
  • Building relationships with partners and identified support agencies
  • Commitment to the principles of Housing First.
  • Adherence to Organisational Health & Safety Policies
  • Participation in Out of Hours On Call Support Rota



Requirements

  • A minimum of two years experience working in the homeless sector or a related field including at least one year coordinating support plans for individuals with complex needs.

  • Strong understanding of Housing First principles including the causes of homelessness and the barriers faced by people at risk.

  • Demonstrated experience working collaboratively with statutory and voluntary agencies to deliver coordinated person-centred support.

  • Sound knowledge and practical application of case management approaches with the ability to work autonomously and manage a varied caseload.

  • Experience in lone working or off-site working is desirable.

  • Commitment to professional development including participation in team meetings supervision appraisals and all mandatory and role-relevant training.

  • Ability to uphold confidentiality professional boundaries and ethical practice in line with organisational policies and codes of conduct.
