About the Role

The Clinical Nurse Manager 1 (Community Liaison Nurse) leads person-centred community-based support planning for autistic adults some of whom also have an intellectual disability across Gheels residential supported living day and community support services. The nursing contribution is expertise planning teaching and advocacy put to work through other people.

This is not a post that delivers clinical interventions. You will not carry a clinical caseload or run a treatment service. Where a person needs clinical care you make sure it is provided by the right primary community or acute health service and that Gheels teams are prepared to support the person through it.

You will be the visible nursing presence in your region known by the people we support working alongside social care colleagues in the places where people live and spend their days so that health support holds up confidently even when no nurse is present.

What Youll Be Doing

The post is built around five core areas of responsibility:

• Support Planning and Person-Centred Practice leading the health and wellbeing elements of each persons support plan health action plans and health-related risk assessments

• Building Capability Competence and Confidence in Teams coaching social care practitioners running competency assessments and acting as an internal trainer for medication management and other health programmes

• Community Liaison and Health Access the day-to-day link between Gheel and primary care ensuring annual health checks preparing people and healthcare partners for appointments and supporting hospital admission and discharge

• Clinical Governance Quality Risk and Safety overseeing medication management contributing to risk registers and nursing audit and supporting HIQA inspection readiness

• Coordination Records and Service Development maintaining nursing records tracking and analysing health activity across the region and deputising for the CNM2 as agreed

What Were Looking For

Essential:

• Registered nurse on the active NMBI Register in the Intellectual Disability Nurses General Nurse or Psychiatric Nurses Division (or eligible to be so registered)

• Minimum of 3 years post-registration full-time experience (or aggregate equivalent) in the relevant Division

• Recent experience of nursing practice in a community residential day service or continuing care setting

• Experience of medication management practice including administration audit and error reporting

• Experience of liaison with primary care community health services or acute hospital services

• Experience of developing individualised support plans care plans or health-related risk assessments

• Experience of teaching coaching or assessing the competence of colleagues including colleagues without a clinical background

• Experience of working alongside social care colleagues or in a multidisciplinary team

• Full clean driving licence and access to own vehicle

Desirable:

• A post-registration qualification at QQI NFQ Level 8 or higher relevant to the area of practice

• A recognised management or supervisory qualification

• Experience of nursing practice with autistic adults or adults with an intellectual disability including people who are non-speaking or who use AAC

• Experience of working within a social care neuro-affirmative or rights-based model of practice

• Experience of supporting people through hospital admission and discharge or people who are ageing in place

• Experience of HIQA inspection preparation in designated centres

Salary and Terms

Salary:53468 to 63402 per annum.
