Case Managers

Job Summary

We are seeking an experienced and highly organised Case Manager to oversee and coordinate a complex care package in Dublin. The role will ensure the delivery of safe effective and person-centred support while managing the day-to-day operation of the package in line with best practice and regulatory standards

Key Responsibilities

• Provide overall leadership coordination and oversight of the care package to ensure consistent high-quality service delivery
• Develop implement and regularly review weekly support plans and staffing schedules responding to changing needs
• Liaise with relevant stakeholders including healthcare professionals support staff family members and external agencies
• Oversee medication management processes including ordering collection storage and administration protocols in line with HIQA standards and organisational policies
• Coordinate shopping household management and daily living supports in accordance with the individuals assessed needs and preferences
• Act as the primary point of contact for family ensuring appropriate access communication and involvement
• Supervise support and guide care staff including induction ongoing supervision and performance management
• Ensure compliance with HIQA standards safeguarding policies and all relevant legislation
• Monitor and manage allocated budgets and resources effectively
• Maintain accurate up-to-date records and reporting in line with regulatory and organisational requirements

Requirements

• Proven experience in complex care disability services or case management
• Strong organisational leadership and interpersonal skill
• Experience overseeing care delivery medication processes and compliance framework
• Ability to work independently while coordinating a multidisciplinary team
• Full driving licence and access to a vehicle (desirable depending on package location
