Tallaght University Hospital (TUH) is a model 4 voluntary academic teaching hospital partnered with Trinity College and situated in south west Dublin. The overall purpose of the candidate Advanced Practice post is to provide safe timely evidenced based physiotherapy-led care in a holistic and autonomous manner to service users at an advanced physiotherapy level. This involves undertaking and documenting a complete episode of care according to collaboratively agreed protocols and scope of practice in the clinical setting demonstrating advanced clinical and theoretical knowledge critical thinking clinical leadership and decision-making skills.
The candidate Advanced Practice Physiotherapist role demonstrates a high degree of knowledge skill and experience that is applied within the Physiotherapist-service user relationship to achieve optimal outcomes through advanced critical analysis problem solving and decision-making.
The appointed candidate is required to successfully complete their candidacy within a maximum of 3 years from commencement of this post. Subject to successful completion of the requirements of the candidacy programme within the identified three year timeframe the successful candidate will then be permanently appointed to the post of Advanced Practice Physiotherapist as set out in section titled Tenure

Overview of role:
Details of the Service:
Musculoskeletal (MSK) Service

Since 2012 the National Clinical Programme for Trauma and Orthopaedic Surgery and the National Clinical Programme for Rheumatology have led the design development and national implementation of therapist led speciality musculoskeletal (MSK) services within Ireland. This model of care is aligned with Sláintecare principles providing timely appropriate safe integrated evidence-based interventions for the management of patients presenting with musculoskeletal conditions that do not require a specialist opinion. The service aims to reduce the referral rate to orthopaedic and rheumatology out-patient departments by offering earlier access to conservative management thus improving functional outcomes and reducing the burden on the healthcare system.

Operating across primary and secondary care settings this service delivers integrated person-centred care closer to home by supporting individuals to manage a wide range of MSK conditions through expert clinical assessment education personalised treatment plans (individual or in a group setting) and self-management strategies. The service facilitates appropriate and efficient use of healthcare resources by triaging referrals to the most appropriate intervention/pathway reducing unnecessary investigations and streamlining access to orthopaedic/rheumatology consultants when required.

Key components of the service include:
  • Advanced clinical assessment and triage of appropriate MSK referrals.
  • Earlier access to the most appropriate evidence-based therapist led interventions.
  • Promote health and well-being through strategies such as Making Every Contact Count (MECC).
  • Co-design of condition specific integrated care pathways with patients GPs orthopaedic/rheumatology consultants health and social care professionals primary care teams IT advocacy groups and voluntary agencies.
  • Sustainable data collection audit and research to inform service development and demonstrate impact.
This service plays a critical role in enhancing patient outcomes reducing work absenteeism improving access to care reducing OPD scheduled care waiting times and ensuring only those that require an orthopaedic/rheumatology consultation are scheduled for same

Key Duties and Responsibilities
A candidate Advanced Practice Physiotherapist will develop competency at a higher level across the four pillars of advanced practice as defined in the HSE HSCP Advanced Practice Framework (2023).

Clinical Practice
A candidate Advanced Practice Physiotherapist will be supported to develop their skills and knowledge to an advanced level. The candidate Advanced Practice Physiotherapist will
  • Practise in compliance with their respective code of professional conduct and within their scope of practice being responsible and accountable for their decisions actions and omissions at this level of practice.
  • Develop and demonstrate a critical understanding of their broadened level of responsibility autonomy and the limits of own competence and professional scope of practice including when working with complexity risk uncertainty and incomplete information.
  • Develop capabilities and competence to practice autonomously and as part of a team using advanced knowledge skills critical thinking and evidence.
  • Develop and implement the highest quality clinical practice. Be supported to create a culture of continuous improvement and innovation.
  • Develop capabilities to create and implement comprehensive service user management as part of a multi-professional team using advanced level clinical reasoning shared service user decision making and evidence based clinical knowledge and skills to achieve optimum outcomes for service user and the service/organisation.
  • Develop and demonstrate a high level of knowledge in relation to patterns of disease or disorder markers of condition progression differential diagnosis and range of treatment available at each stage of disorder or condition including recognition of red flags and potentially complex and serious presentations and the need for expedited onward referral to the appropriate team.
  • Develop and demonstrate a high level of awareness and recognition of the psycho-social impact of disease and disorders on service users and their families.
  • Under supervision undertake specialist assessment of service users in partnership with individuals families and carers using a range of methods and tools informed by theory and evidence.
  • Develop their expertise and decision-making skills to inform clinical reasoning when dealing with differentiated and undifferentiated individual presentations and complex situations synthesising information from multiple sources to make appropriate evidence-based judgements and/or diagnoses. This will involve developing and demonstrating competence in planning implementing and evaluating the care delivery according to service user needs.
  • Under supervision play a direct role in the management of complex service user as part of a multi-professional team or autonomously including assessment of the service users relevant history developing an investigation strategy including where appropriate referring for or conducting tests interpreting results where appropriate to the profession and agreeing a management and treatment plan in partnership with the service user and family carers medical staff and the multi professional team.
  • Where appropriate to profession and role develop the skills to take a history assess examine diagnose prescribe and develop a management plan including medication and /or diagnostic imaging and monitor response to medication/treatment in HSE Policy.
  • Develop and demonstrate a clear understanding of the legal context for anti-oppressive practice promotion of human rights and social justice in diverse health and social care settings and an ability to balance the risks and rights of the service user liaising with and referring to their relevant health and social care professionals as required.
  • Develop advanced understanding and ability to navigate healthcare settings. Advise and communicate as appropriate with acute hospitals primary and social care and community teams thus ensuring seamless continuity and transfer of care for service users between other relevant healthcare professionals health and social care services third sector agencies and other care settings.
  • Develop capabilities to establish maintain and effectively manage barriers to advanced highly skilled and effective communication with service users carers and healthcare professionals. This includes imparting information regarding diagnosis prognosis and treatment of complex conditions and referring to other MDT teams as appropriate to promote integrated working and to improve service user outcomes.
  • Develop and demonstrate an advanced ability to enhance and promote the rights of a person to actively participate in their healthcare management through shared decision making by taking into consideration the service users wishes goals attitudes beliefs and circumstances.
  • Develop and demonstrate shared decision making with all individuals involved in determining and managing goals clinical interventions social prescribing and measurable outcomes to ensure integrated service user care to serve the individuals best interest.
  • Develop and demonstrate a knowledge of digital transformation in healthcare through the application of digital technologies to enhance or modify service user focused care.

The candidate Advanced Practice Physiotherapist will:

The candidate Advanced Practice Physiotherapist will

The candidate Advanced Practice Physiotherapist will:
  • Develop skills to critically evaluate and synthesise the evidence available and make appropriate judgements on implementation in practice despite incomplete or paradoxical evidence.
  • In collaboration with other staff/colleagues ensure that the approach to all levels of clinical practice is person-centred evidence based in accordance with professional practice national guidelines/models of care and national/local/international benchmarks.
  • Develop capabilities to lead and develop a culture of enquiry that promotes supports and encourages participation in evidence-based practice including research service evaluation quality improvement (Q.I.) and audit.
  • Take a critical approach to identify gaps in the evidence base and its application to practice alerting appropriate individuals and organisations to these and how they might be addressed in a safe and pragmatic way and where possible also plan and deliver the research to address these gaps.

Risk Management Quality Health & Safety
The candidate Advanced Practice Physiotherapist will:
  • Exercise professional judgement in development of policies/ procedures/ guideline and protocols to support risk management as appropriate and as required especially where there may be complex and unpredictable events and supporting teams to do likewise to ensure safety of individuals families and carers.
  • Adequately identify assess manage and monitor risk within their area of responsibility.
  • Have a working knowledge of the Health Information and Quality Authority (HIQA) Standards as they apply to the role for example Standards for Healthcare National Standards for the Prevention and Control of Healthcare Associated Infections Hygiene Standards etc. and comply with associated HSE protocols for implementing and maintaining these standards as appropriate to the role.
  • Support promote and actively participate in sustainable energy water and waste initiatives to create a more sustainable low carbon and efficient health service.
  • Engage in the HSE performance achievement process in conjunction with Line Manager and staff as appropriate.
  • Be familiar with and responsible for attending the necessary education training and support to enable them to meet this responsibility.

  • Have a working knowledge of PPPGs in relation to the care and safety of any equipment supplied for the fulfilment of duty within the cAP Physiotherapy role. Ensure the advice of relevant stakeholders is sought prior to procurement.
Qualifications & Experience required
Must have:
  • Be registered or be eligible for registration on the Physiotherapists Register maintained by the Physiotherapists Registration Board at CORU.
And
  • Have five years full time (or an aggregate of five years) post registration
    qualification experience. Of which four years full time (or an aggregate of four
    years) post registration qualification clinical experience must be in the required
    area of Specialism
  • Demonstrate a proven record of clinical excellence in the specialism
  • Candidates must demonstrate evidence of continuing professional development relevant to the required area of specialism in the form of post-graduate qualifications or relevant courses
  • Candidates must demonstrate achievement in the areas of clinical audit quality improvement initiatives practice development teaching and research
  • Provide proof of Statutory Registration on the Physiotherapists Register
    maintained by the Physiotherapists Registration Board at CORU before a
    contract of employment can be issued.
  • Candidates must possess the requisite knowledge and ability (including a high standard of suitability management leadership and professional ability) for the proper discharge of the duties of the office.

Desirable:
  • Proven experience in the management of low back pain.

Reward & Recognition
  • Remuneration is in accordance with the Department of Health Consolidated Salary Scales grade code 3232.
  • The appointment is Full Time Specified Purpose and pensionable.
  • The annual leave entitlement is 29 working days per year. The leave year runs from 1st April to the 31st of March each year.
  • Normal working hours are 35 worked over 5 days. Your contracted hours of work are liable to change between the hours of 8 am to 8 pm over 7 days to meet the requirements for extended day services in accordance with the terms of the Framework Agreement.

SUPPLEMENTARY INFORMATION

Age Restrictions In Relation To Applications
Age restriction shall only apply to a candidate where he/she is not classified as a new entrant
(within the meaning of the Public Service Superannuation (Miscellaneous Provisions) Act 2004).
A candidate who is not classified as a new entrant must be under 65 years of age on the first day
of the month in which the latest date for receiving completed applications for the office occurs.

Health
A candidate for and any person holding the office must be fully competent and capable of undertaking the duties attached to the office and be in a state of health as would indicate a reasonable prospect of ability to render regular and efficient service.

Garda Vetting
Garda vetting is sought for all employees and prospective employees of Tallaght University Hospital. Given the specialised nature of the services provided your appointment will be subject to satisfactory Garda vetting and re-vetting in circumstances where the Hospital deems it appropriate. The Hospital will then process the necessary documentation to endeavour to obtain the satisfactory Garda clearance for you. You are obliged to disclose previous and any criminal convictions acquitted during the course of your employment. Should the Hospital obtain information from the Garda vetting unit to indicate that your Garda clearance report is not satisfactory and / or if you have supplied the Hospital with false or misleading information in relation to your Garda clearance status the Hospital reserves the right to withdraw or terminate the contract in accordance with Tallaght University Hospital Garda vetting policy.


Point on Salary Scale
New appointees to any grade start at the minimum point of the scale. Incremental credit will be applied for recognised relevant service in Ireland and abroad (Department of Health Circular 2/2011). Incremental credit is normally granted on appointment in respect of previous experience in the Civil Service local authorities health service and other public service bodies and statutory agencies.

Probation
The probationary period may be extended at the Hospitals discretion but will not in any case exceed 9 months. Confirmation of your appointment as a permanent member of staff is subject to the successful completion of the probation period. Termination of this agreement within the probationary period shall be at the discretion of the Hospital in accordance with the Minimum Notice and Terms of Employment Act 1973 - 2001.

Termination of Employment
The employment may be terminated at any time by one months notice on either side except where circumstances are dictated by the Minimum Notice and Terms of Employment Act 1973/91. The Managements right under this paragraph shall not be exercised save in circumstances where the Management is of the opinion that the holder of the office has failed to perform satisfactorily the duties of the post or has misconducted himself/herself in relation to the post or is otherwise unfit to hold the appointment.

Sick Leave
  • There is provision for sick pay at the Hospitals discretion subject to the regulations and details found in the Staff Handbook/ Personnel Policy Manual as per the Hospitals sick leave policy.
  • In the event of absence from work you are required to contact your Manager/Supervisor within one hour of starting time on the first day of absence while a Medical Certificate is required for any illness lasting in excess of two days. Failure to comply with the sick leave policy may result in a loss of pay for the period of absence and may also incur disciplinary action.
Information Technology & data
  • Make the most effective and efficient use of developments in information technology for both patient care and administrative support in a manner which integrates well with systems throughout the organisation.
  • Collect interpret and present data and information on the Departments activities staffing and expenditure as required.
Pension Arrangements and Retirement age
There are three superannuation schemes currently in operation for staff in Tallaght University Hospital:
(a) Voluntary Hospital Superannuation Scheme (Non-New Entrant)
(b) Voluntary Hospital Superannuation Scheme (New Entrant)
(c) Single Public Service Pension Scheme

Please read carefully the summary of the criteria for the three superannuation schemes below. This will assist you in identifying which scheme membership is applicable to your employment circumstances.
(a) If you have been continually employed in the public service prior to the 1st April 2004 and you have not had a break in service of more than 26 weeks you will be included in the Voluntary Hospital Superannuation Scheme (Non-New Entrant) with a Minimum Retirement Age of 60 and a Maximum Retirement Age of 65.
(b) If you have been continually employed in the public service post the 1st April 2004 and you have not had a break in service of more than 26 weeks you will be included in the Voluntary Hospital Superannuation Scheme (New Entrant) with a Minimum Retirement Age of 65. There is no Maximum Retirement Age.
(c) If you have commenced employment in the public service as a new entrant or you are a former public servant returning to the public service after a break of more than 26 weeks you will be included in the Single Public Service Pension Scheme effective from the 1st January 2013 with a minimum retirement age of 66 (rising to 67 and 68 in line with state pension changes). The maximum retirement age under this scheme will be age 70.

Hygiene / Infection prevention and control
  • All employees have responsibility for Hygiene awareness. Hygiene is defined as The practice that serves to keep people and environments clean and prevent infection.
  • All employees also have a responsibility under their terms of employment to prevent transmission of infection and to follow the hospital infection control policies and guidelines as outlined in the Infection Control Manual.
  • All employees must be aware that they work in an area where there is potential for transmission of infection.
  • All employees have a responsibility to follow hand hygiene guidelines as this is the single most important intervention to prevent the transmission of infection.
Quality safety and risk management
  • Support the delivery of the Quality Safety and Risk Management Programme including the appropriate identification and management of risks and incidents throughout the hospital.

Mandated and Designated Persons under Children First Act 2015
Schedule 2 of the Children First Act 2015 specifies the classes of persons defined as Mandated Persons for the purposes of the Act. This includes a range of disciplines that are employed by the hospital including all medical practitioners registered nurses physiotherapists psychologists social workers and others. This includes staff working in adult services. All mandated persons have two main legal obligations under the Children First Act 2015. Mandated persons under the legislation are required to report any knowledge belief or reasonable suspicion that a child has been harmed is being harmed or is at risk of being harmed. The Act defines harm as assault ill-treatment neglect or sexual abuse and covers single and multiple instances. Mandated persons are also required to assist Tusla if requested in assessing a concern which has been the subject of a mandated report. Designated Officer are required to receive reports of suspected child abuse or neglect from any person who is of the opinion that (a) a child has been or is being assaulted ill-treated neglected or sexually abused or (b) a childs health development or welfare has been or is being avoidably impaired or neglected.
