On-Call Inpatient Medical Social Worker

Job Title: OnCall Inpatient Medical Social Worker
Location: Honolulu HI US
Employment Type: Permanent Onsite
Shift: Day shift 8a4:30p Must work at least 3 shifts per month weekdays and weekends
Salary: USD $35.00 $41.00 / hr

Essential Responsibilities:

• Prepares and evaluates assessments development and implementation of social work service plans.
• Provides psychosocial assessment diagnostic impressions and treatment for individuals/couples/families/groups.
• Performs case management coordination casework intervention and monitoring of social work service plans.
• Assists with administration development implementation and evaluation of social work programs and policies.
• Conducts research and documents according to departmental standards Federal and State regulations.
• Participates in departmental functions such as staff meetings staff inservices and committees as assigned.

Qualifications:
Basic Qualifications:

• Education: Masters degree in social work.
• License Certification Registration: Licensed Social Worker (Hawaii) OR Licensed Clinical Social Worker (Hawaii) Basic Life Support required at hire from American Heart Association.
• Additional Requirements: Healthcare experience working with medical diagnoses demonstrated ability in networking with community agencies.

Preferred Qualifications:

• Minimum one (1) year social work experience in a hospital medical setting home health etc.
• Experience with advanced care directives.
• Experience in insurance referrals.
