Job Description LPN (Licensed Practical Nurse)

Our client a reputable rehabilitation and nursing center is actively seeking Licensed Practical Nurses (LPNs) to join their dedicated team on a PRN/as needed/per shift basis. This presents an excellent opportunity for nurses looking to augment their schedules with flexibility. Position Summary:
The successful LPN candidate will thrive in a dynamic fastpaced rehabilitation and nursing center. Adaptability is key as the role requires the ability to quickly familiarize oneself with various software applications.
Responsibilities:
- Conduct comprehensive evaluations of residents rehabilitation needs.
- Collaborate with medical professionals to plan and execute resident care in alignment with physician orders.
- Implement and oversee resident care plans regularly evaluating and updating as necessary (PRN).
- Administer medications to residents according to prescribed schedules.
- Monitor vital signs including blood glucose levels temperature and blood pressure.
- Assist residents with Daily Living Activities including bathing dressing grooming eating transfer ambulation toileting and other personalized care needs.
- Supervise and ensure Certified Nursing Assistants (CNAs) adhere to physician orders.
Qualifications:
- Must be currently licensed in the State of New York as a Licensed Practical Nurse.
- Must be CPR Certified.

Experience & Education High School diploma or equivalent required. 2 years of experience in a healthcare or Managed Care setting required. An equivalent combination of education and experience sufficient to successfully perform the essential duties of the position such as those listed above is also qualifying. Preferred Qualifications Bilingual in English and in one of our defined threshold language (English, Spanish, Vietnamese, Farsi, Arabic, Korean, and Chinese) preferred. Prior Authorization and Utilization Management process knowledge preferred. Knowledge of: Medical Terminology. Medi-Cal and Medicare benefits and regulations. Current ICD 10- CPT, and Healthcare Common Procedure Coding System (HCPCS) Coding continual updates to knowledge base regarding the codes.
