Job Summary

Ensures the safe and effective use of medications throughout the Hospital in compliance with State and Federal regulations and consistent with current best practices in evidencebased medicine. Acts as a medication information resource to patients medical staff nursing staff and ancillary department personnel. Provides high quality comprehensive pharmaceutical care appropriate to the age of the patient which contributes to positive patient outcomes.

• Consult other departments as appropriate to provide for an interdisciplinary approach to the patients needs.

• Ensures patient confidentiality.
• Maintain a good working relationship both within the department and with other departments.
• Ensure that documentation meets current standards and policies.
• Manage and operates equipment safely and correctly.
• Coordinate and supervises patient care as necessary.
• Plans organizes and directs Pharmacy operations and ensures compliance with all local state and federal regulations.
• Develops implements and communicated policies guidelines and standards for the Pharmacy Department in accordance with current standards.
• Prevent detect and resolve medicationrelated problems and make appropriate evidencebased patientcentered medication therapy recommendations as part of the interdisciplinary team.
• Accurately review and verify medication orders resolving medication therapy interactions allergies and incompatibilities.
• Compound medications using standard formulas and processes such as weighing measuring and mixing ingredients.
• Participated on hospital committees such as the Pharmacy and Therapeutics Committee Antimicrobial Stewardship Committee Department Director Meeting Infection Control Meeting and Quality Committee.
• Prepares department budget for personnel operating expenses and capital equipment. Responsible for fiscal operation of the department.
• Responsible for the implementation monitoring and evaluation of quality improvement initiatives.
• Maintains formularies information resources and references appropriate for the scope of services.
• Prepare IV admixtures and extemporaneous parenteral products demonstrating proper technique.
• Ensure accuracy and appropriateness of medication distributed throughout the hospital system.
• Provide concise applicable comprehensive and timely responses to requests for medication information.
• Reports medication errors and adverse drug events and effectively contributes to the timely resolution of current event and prevention of future events.
• Provide leadership and oversight for Pharmacy Technicians and Interns.
• Provide and document patient medication education per protocol.
• Monitoring and controlling inventory to ensure medication availability quality of products available and minimizing waste.

Qualifications

Qualifications

Current registered pharmacist in the state of New Mexico
Bachelors or Doctorate in Pharmacy.
