JOB SUMMARY

The Occupational Therapist provides occupational therapy to patients who require restoration of physical and mental functioning in order to gain independence in self-care and other May supervise Certified Occupational Therapy Assistants and QUALIFICATIONS Certificate or degree from an accredited occupational therapy Licensed by the state in which the Occupational Therapist resides or practices, if state requires, and has received certification from the National Board for Certification in Occupational Therapy, (NBCOT).

QUALIFICATIONS
Certificate or degree from an accredited occupational therapy Preferably two years of clinical experience (if applicable). Licensed by the state in which the Occupational Therapist resides or practices, if state requires, and has received certification from the National Board for Certification in Occupational Therapy, (NBCOT).

Requires moderately heavy occupational exertion on a regular and reoccurring basis such as extensive driving, assisting patients in transfer activities (wheelchair to bed, to tub, to commodes) and providing substantial support to individuals in ambulation Regularly required to use hands and arms, stoop, kneel, crouch or Occasionally required to Occasionally lift and/or move over 100 Specific vision abilities include close, peripheral, depth perception and ability to adjust

WORKING CONDITIONS

ESSENTIAL FUNCTIONS

Provides treatment as ordered by the attending

Assists the physician in evaluating the patient s level of Evaluates and establishes goals based on the patient s current level of function and potential Assists in developing and updating the plan of

Guides and instructs therapeutic, self-care and creative activities that are directed toward improving independence and physical and mental

Observes, records and reports to the physician the patient s reaction to the therapy program or any changes in the patient s

Advises, consults and instructs patients in the use of prosthetic, orthotic and assistive Instructs, teaches and supervises other health care team personnel in the case of patients requiring occupational

May supervise Certified Occupational Therapy Assistant(s) and co-sign Certified Occupational Therapy Assistant s progress

Adheres to all applicable federal, state, local and company maintained standards of care and ethics and policies concerned with the practice of occupational therapy and among other things, personnel

Participates in staff development Participates in in-services education

Demonstrates evidence of continuing education conducted for the in-service

Meets productivity and utilization expectations as established by the Company and the

1 Covers more than one facility as required by the need of the
