  • Manages employees in the provision of care and services rendered in accord with professional standards and in compliance with state and federal laws and regulations
  • Directs the management of clinical and nonclinical departments to ensure clients employees contractor s satisfaction
  • Provides leadership and support related to compliance activities; monitors and ensures Compliance with all regulatory standards.
  • In collaboration with the AVP of Home Care Services works with upper management to establish overall growth and new development strategies and translates those strategies into operational plans. Establishes performance metrics by business unit and reports outcomes to Senior Management.
  • Assists the Director of Home Care Services in directing the management of clinical and nonclinical departments to ensure clients employees contractor satisfaction costefficient processes and outstanding home care services.
  • Works with the AVP and Director of Home Care Services in collaborating with consultants contractors referring physicians community resources government agencies and advocacy groups.
  • Assists in promoting Best Choice and PACE programs to the community major customers government agencies and organizations. Assists in developing and maintaining relationships with leaders of other healthcare organizations.
  • Assists in enhancing and/or developing implementing and enforcing policies and procedures of the organization by way of systems that will improve the overall operation and effectiveness of Homecare services.
  • Obtains and maintains current information and knowledge of the Home Care environment and evaluates the appropriate strategies and tactical approaches to that environment
  • Oversees coordinates and provides support to sitebased Managers and clinical staff; inclusive of but not limited to the DTC and Day Health Center rehab department nutritional services social workers and home services management.
  • Assists with leading Care Planning meetings ensures all meetings are timely monitors any necessary followup after meetings and serves as a mediator when teams have disagreements regarding patient care treatments.
  • Balances & allocates IDT and related resources throughout the site and ensures adequate team assignments and coverage.
  • Collaborates with the Care Planning Team to ensure the care plans for each participant are completed timely.
  • Provides oversight of the provision of services that impact participants care.
  • Implements inservice training and management directives to support the development and advancement of team members.
  • Collects and reviews data for operational metrics geared towards ensuring participants are treated in the right venue at the right time and at the right cost at the highest levels of quality.
  • Participates in all quality assurance and regulatory compliance activities to ensure appropriate regulatory certifications.
  • Collaborates with departments within the clinical development pillar (medical board research & innovation clinical standards and processes) to establish new & improved care delivery processes and treatment protocols.
  • Monitors and minimizes voluntary disenrollments
  • Investigates and assists in documenting and resolving customer issues and grievances in a timely accurate and courteous manner. This includes working with the other departments to resolve the source of the problem(s).
  • Ensures all documentation for all participants is complete and accurate including and not limited to enrollment disenrollment electronic/paper medical record and any other clinical documentation and address any deficiencies.
  • Participates in all quality assurance and regulatory compliance activities to ensure appropriate regulatory certifications.
  • Ensures the timely facilitation by the IDT in the implementation of the core function of effective care management: ensuring that the IDT provides the assessment planning care coordination implementation advocacy and evaluation/oversight necessary on all aspects of health care delivery to CLHC PACE Participant.
  • Ensures Level 2 investigation is completed in a timely manner according to regulation.
  • Ensures that direct reports conduct their duties according to applicable rules and regulations as well as the organization s policies and procedures.
  • Evaluates the work performance and competency of direct reports. This includes ongoing informal feedback/counseling/support and completion of annual performance reviews.
  • Follows the organization s policies regarding disciplinary action. Engages Human Resources as needed for guidance on disciplinary actions and terminations.
  • Develops individual professional development plans with direct reports including the identification of inservice training needs and interests.
  • Investigates and resolves employee grievances and complaints. Engages with Human Resources as appropriate.
  • Participates in the annual budgeting process for the department; assists in managing departmental budget; assists in approving departmental expenditures.
  • Participates in special projects and assignments as required
  • All other duties as assigned.

Requirements


Education:
Bachelor s Degree in healthcare administration nursing business or other healthcarerelated fields required. Master s Degree in Health Care Administration preferred. Completed NYSED approved infection control coursework. A minimum of 8.0 education contact hours that may include contact hours of mandatory training requirements such as infection control coursework

Case Management Certification preferred.


Experience: Minimum of 3 years of administrative experience in a management capacity in a certified home health agency (CHHA) longterm home health care licensed homecare service agency (LHCSA) acute care medicalsurgical and/or critical care nursing home experience diagnostic & treatment clinic preferred.

Customer Service experience required.

Managed longterm care insurance experience beneficial.

Minimum of 3year experience working with the elderly


Other: Exemplary leadership organizational interpersonal and communication skills experience with a large multiservice healthcare provider is necessary as is demonstrable knowledge/competence in community health principles practices and administration


License Registration: Current active and unrestricted license and registration in New York State required as a Registered Nurse.


Language: Bilingual preferred.


Physical Requirements

Individuals must be able to sustain specific physical requirements essential to the job. This includes but is not limited to:

  • Standing duration of up to 6 hours a day
  • Sitting/Stationary positions sedentary position for a duration of up to 68 hours a day for consecutive hours/periods.
  • Lifting/Push/Pull up to 50 pounds of equipment baggage supplies and other items used in the job scope using OSHA guidelines etc.
  • Bending/Squatting have to be able to bend safely or squat to perform the essential functions under the scope of the job.
  • Stairs/Steps/Walking/Climbing must be able to maneuver stairs safely climb up/down and walk to access work areas.
  • Agility/Fine Motor Skills Must demonstrate agility and fine motor skills to operate and activate equipment devices instruments and tools to complete essential job functions (i.e. typing use of supplies equipment etc.)
  • Sight/Visual Requirements must be able to read documentation papers orders signs etc. and accurately type/write documentation etc.
  • Audio Hearing and Motor Skills (language) Requirements must be able to listen attentively and document information from patients community members coworkers clients providers etc. and intake information through audio processing with accuracy. In addition one must be able to speak comfortably and clearly with language and motor skills for customers to understand an individual.
  • Cognitive Ability Must be able to demonstrate good decisionmaking reasonableness cognitive ability rational processing and analysis to satisfy essential functions of the job.





Benefits

Weekly Hours: 40 hours

SCHEDULE: 8:30 AM 5:30 PM

Salary :$120k $135k
