Advanced Practice Nurse

Immediate Need for an Advanced Practice Nurse located in Brooklyn NY to support a leading home healthcare provider. The Advanced Practice Nurse will act as a key resource in providing clinical and operational guidance and support to patients families providers and internal team membersResponsibilities:Collaborate with physicians and develop clinical protocols Provide comprehensive advanced nursing care including assessments interventions and patient education Collaborate with pertinent professionals family and team members to complete assessments and care planning Ensure compliance with quality of care measurements and standards Assess educate and improve patient and caregiver knowledge of chronic disease management Communicate with internal and external care partners for timely interventions Manage quality of medical record documentation and billing information Qualifications:License and current registration to practice as a Registered Professional Nurse and a Nurse Practitioner in the State of New York with a specialty in adult health family health or gerontology 5 Years of overall clinical experience 2 Years of experience as a Nurse Practitioner utilizing full scope of practice Clinical home care experience or 2 years of managerial experience preferred NYS Valid drivers license as determined by operational/regional needs may be required Maintains credentialed status with clients Medical Care at Home and associated managed care plans along with NPI Medicaid and Medicare provider numbersMasters Degree of Science in Nursing from a nurse practitioner educational program registered by the New York State Education Department as qualifying for NP certification (licensure) Current ANCC or AANP certification as an adult family or geriatric nurse practitioner Demonstrated knowledge of HEDIS and QARR quality measures ICD9 and CPT coding for reimbursement of services required Bilingual Spanishspeaking skills are preferred but not required
