The Vice President of PostAcute and Palliative Care Services is responsible for overseeing and managing various postacute care assets including home care home hospice palliative care and inpatient rehab facilities (IRFs). The role leads a division with over 1000 Full Time Equivalents. The role involves strategic leadership growth initiatives collaboration with internal and external stakeholders ensuring compliance with regulatory standards and ensuring quality and excellent consumer experience.
Key Responsibilities:
- Strategic Leadership and Growth:
- Spearhead the communication and implementation of effective growth strategies to enhance postacute care offerings.
- Foster a growth mindset within the team.
- Collaboration and Coordination:
- Collaborate with system enterprise leaders to improve patient transitions between acute and postacute care settings.
- Coordinate with Clinically Integrated Network ambulatory care health plan and other Sentara Divisions to streamline care pathways.
- External Relationships:
- Cultivate and develop a highperforming postacute care network with external postacute care delivery organizations including LTCHs SNFs IRFs Home Care and Hospices.
- Regulatory Compliance:
- Ensure full compliance with federal state and local regulatory standards across postacute care services.
- Quality Improvement and Innovation:
- Implement innovative solutions to enhance care delivery and optimize patient outcomes.
- Drive improvements in QualityofCare outcomes aiming for top decile performance in CMS star ratings and other valuebased incentive programs.
- Focus on enhancing care quality reducing readmissions and optimizing patient transitions through the broader care continuum.
- Consumer Experience:
- Ensure that the consumer experience is seamless efficient and patient centric for the patient and families as measured by consumer surveys and community surveys.

Qualifications: MD Doctor of Medicine: Doctor Of Osteopathic Medicine MLD Masters Level Degree: Health Administration MLD Masters Level Degree (Required)
