FACILITY NEEDS AN ASAP START AND WILL NOT CONSIDER START DATES AFTER 4/15!

Responsible for the transition of care/discharge planning for both inpatients and our skilled rehab patients.

  • Triages coordinates and facilitates the clinical management for an assigned population.
  • Develops monitors and reinforces care standards and protocols designed to enhance quality and promote costeffective utilization of healthcare resources.
  • Manages the care of assigned patients from intake through discharge and follow up.
  • Coordinates the delivery of services to patients and families.
  • Manages the financial resources of the patient by acting as the liaison to the payer source and assuring costeffectiveness of the treatment provided by the team.
  • Facilitates and coordinates team conference reporting and the discharge planning process.
  • Serves as the patient and family advocate to coordinate and direct effective communication regarding the patient s care.
  • Performs postdischarge followup.

Desired qualities and behaviors include: collaborative team player with strong communication skills use of a systems approach in planning problem solving and decision making creativity innovation risk taking autonomy flexibility receptiveness to change and a commitment to professional growth. Computer skills required.


8hrs nonbillable orientation
Holiday and OT multipliers: 1.25%

  • Minimum 3 years clinical experience
  • Recent acute care hospital experience
  • BSN preferred
  • Previous travel preferred
  • Utilization review experience a plus

  • Cover page (if available/not required)

  • Skills Checklist (current to 1 year)
  • 1 Supervisory Signed Reference from within the past two years of employment
  • License/Certifications Must be uploaded with profile

CERTIFICATION REQUIREMENTS :
  • BLS (AHA)

STATE LICENSE REQUIREMENTS :
  • Vermont
