Responsibilities:
Supervise all Hospice and Home Health Social Workers.
Implementation of standards of care for social work services
Conducts interdisciplinary patient assessments and addresses psychosocial needs.
Participates in the coordination of services with other clinic team members and community resources as needed
Manages a patient caseload as needed providing counseling crisis intervention and social evaluations.
Requirements:
Clinical Social Worker State License
Masters degree in Social Work (MSW)
Clinical experience practicing as a LCSW

- Management experience
Hospice and/or Home Health experience preferred
Compassion and integrity
