Social Worker MSW

The Social Worker (MSW) is responsible for nonclinical biopsychosocial assessments and interventions for the inpatient floors and the outpatient emergency department. The MSW performs crisis intervention patient/family interventions highrisk screening focused emotional support and referrals for financial assistance or other identified resource needs. The MSW assists in postacute care placement on all discharges primarily with those patients identified with postdischarge behavioral health and patient assistance/resource needs. The MSW is dedicated to patient and familycentered care that values personal selfdetermination and prioritization and engages in creative and compassionate problemsolving. The scope of this role includes intensive care units Trauma Oncology NICU Transplant and ED. The MSW responds to physician nondischarge planning social work consults throughout the facility.

Job Responsibilities

• You will work collaboratively with RN case managers acute care nurses physicians insurance companies and other staff members to proactively identify and address patient needs.
• You will identify and address barriers to discharge and recovery. These barriers include financial emotional psychosocial medical and spiritual.
• You will maintain a thorough working knowledge of the rules of Medicare Medicaid and privatepayer regulations and processes.
• You will possess a thorough understanding of managedcare concepts and lengthofstay management relating to patient and organizational fiscal responsibility.
• You will perform thorough assessments and develop appropriate discharge plans for highrisk patients with discharge planning or psychosocial needs.
• You will coordinate and facilitate meetings between the care team and patients/families/caregivers. You will assist patients and families in making difficult decisions while honoring their preferences and values.
• You will intervene with patients/families/caregivers for psychosocial needs assistance and support in coping with chronic acute terminal illness. You will engage in consultation discharge planning education counseling advocacy and information referrals to meet the emotional social and financial aspects of care.
• You will provide emotional support to patients and family members during complex and/or emotionally charged issues such as endoflife terminal illness notifications death notifications (with physicians) and crisis interventions.

Qualifications

• MUST HAVE Masters Degree in Social Work from an accredited school
• State License Eligible
• A minimum of 1 year of hospital Social Work experience is preferred

2 Positions Social Worker 2 MUST HAVE LMSW RPC WILL OFFER CLINICAL SUPERVISION

Social Worker 3 MUST HAVE LCSW LICENSE

Compensation: Salary range $24.75 $34.65 SW2; Salary range $29.41 $41.16 SW3
