Social Worker MSW

Essential Job Duties:

• Conduct assessments in social work to determine the psychosocial needs preferences and goals of the participants. Actively participate in team meetings to develop care plans for participants.
• Deliver and document interventions in social work as agreed upon in the care plans of participants. This includes arranging necessary resources and services assisting with care transitions providing individual and group counseling as well as case management.
• Collaborate with the primary care physician and other members of the care team to ensure smooth care transitions between different settings such as hospitals skilled nursing facilities and home.
• Initiate coordinate and facilitate care conference meetings to ensure excellent care coordination among other care team members participants and individuals in the participants support network including family and informal caregivers.
• Provide discharge planning when participants disenroll from the program.

Job Requirements:

• Masters Degree in Social Work (MSW) is required.
• Minimum of one (1) year of experience
