Location: New York City Metropolitan Area (OnSite)

We are seeking an experienced RN Home Care Nursing Supervisor who is compassionate dedicated and able to uphold our standards and culture. If you have experience working at a New York State Licensed Home Care Agency providing Supervisory and Assessment visits we want to hear from you.

Qualifications:

• Current and valid NYS RN License
• Minimum 1 year of home care experience doing Supervisory Visits and Assessments (Preferred)
• Ability to accurately assess patients health care status and psychosocial needs
• Effective communication skills with physicians patients and caregivers
• Ability to multitask and communicate effectively both verbally and in writing
• Experience with Microsoft Office and HHA Exchange (A Plus)
• Reliable means of transportation to conduct home visits in at least 3 NYC Boroughs

Responsibilities:

• Conduct inperson initial patient assessments reassessments quality assurance and supervisory visits of HUHCs field staff (paraprofessional caregivers and nurses)
• Assist with developing implementing and revising patients Plan of Care
• Ensure that all HUHC caregivers adhere to the Plan of Care and Physicians Orders
• Provide assistance to DPS and Coordination in recommending ideal caregivers for each patient considering clinical and psychosocial needs
• Communicate effectively with HUHC team members to ensure each patient receives the highest quality home care from HUHCs caregivers
• Additional duties as assigned

Job Type: Fulltime

Work Location: Manhattan office but on the road at least 50% of the time
