JOB DESCRIPTION Must have acute hospital experience in CM and discharge planning Do not submit CMs with only SNF REHAB or Hospice experience Must have prior travel experience Certification is not required. Must be willing to work in weekend rotation REQUIRED 1 yr. minimum recent experience with discharge planning as a Clinical Case Manager with a heavy focus on discharge planning in an acute care hospital setting Need to have acute INPATIENT case manager experience. BLS InterQual EPIC Allscripts/Careport PREFERRED 2 or more years Utilization Review is a plus ACM or CCM Certification is a plus
