We operate with a network of over 150 therapists throughout Michigan. Were thrilled to invite an exceptional Occupational Therapist to join our team in an exceptional subacute care facility. Our mission of this program is to make a positive impact on the lives of our patients through compassionate and effective rehabilitation services.

Responsibilities:

  1. Initial Assessment: Conducting thorough physical assessments to evaluate the musculoskeletal neuromuscular and cardiovascular status of geriatric patients. This includes assessing mobility strength flexibility balance and coordination.

  1. Developing Individualized Treatment Plans: Collaborating with the healthcare team to create personalized treatment plans based on the assessment findings. Setting functional goals and outlining interventions to improve or maintain mobility and physical function.

  1. Exercise Prescription: Designing and implementing tailored exercise programs to address specific impairments and improve the strength endurance and flexibility of geriatric patients. This may include both landbased and aquatic exercises.

  1. Gait Training: Providing gait training and mobility exercises to enhance walking abilities and reduce the risk of falls. Assessing and addressing any abnormalities in the gait pattern.

  1. Balance Training: Implementing balance exercises and activities to improve stability and reduce the risk of falls among geriatric individuals. This may involve both static and dynamic balance training.

  1. Pain Management: Incorporating pain relief techniques and modalities to address musculoskeletal pain or discomfort commonly experienced by geriatric patients.

  1. Joint Range of Motion (ROM) Exercises: Administering exercises to improve and maintain joint mobility addressing stiffness and promoting optimal range of motion in geriatric patients.

  1. Manual Therapy: Utilizing manual therapy techniques such as joint mobilizations and soft tissue mobilization to address musculoskeletal issues and improve overall function.

  1. Assistive Device Training: Educating and training geriatric patients on the proper use of assistive devices such as canes walkers or crutches to enhance mobility and independence.

  1. Patient Education: Providing education on injury prevention home exercise programs and strategies to manage and cope with physical limitations. Educating patients on maintaining a healthy and active lifestyle.

  1. Home Exercise Programs: Developing and prescribing home exercise programs that allow geriatric patients to continue rehabilitation and maintain gains in physical function between therapy sessions.

  1. Monitoring Progress and Adjusting Treatment Plans: Regularly assessing and documenting the progress of geriatric patients. Modifying treatment plans as needed to address changing needs and ensure optimal outcomes.

  1. Collaboration with Other Healthcare Professionals: Collaborating with physicians occupational therapists nurses and other healthcare professionals to ensure a comprehensive and coordinated approach to geriatric care.

  1. Patient Advocacy: Advocating for the wellbeing and functional independence of geriatric patients within the healthcare system ensuring that their unique needs are considered in the overall care plan.
