PACE Primary Care Nurse Practitioner FNP or AGNP

Seasoned Recruitment is currently seeking a dedicated and experienced Primary Care Nurse Practitioner (FNP or AGNP) to join a wellestablished PACE (Program of AllInclusive Care for the Elderly) organization.

As a PACE Primary Care Nurse Practitioner you will provide comprehensive primary care services to elderly individuals enrolled in the PACE program. You will be responsible for conducting assessments diagnosing and treating medical conditions prescribing medications and collaborating with an interdisciplinary team to develop and implement care plans.

Responsibilities:

• Conduct comprehensive physical and psychosocial assessments of elderly individuals
• Diagnose and treat acute and chronic medical conditions
• Prescribe medications and order diagnostic tests
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Provide health education and counseling to patients and their families
• Monitor and evaluate patient outcomes adjusting care plans as necessary
• Coordinate and communicate with other healthcare providers
• Adhere to regulatory and documentation requirements
