Hours: 8a430p Job Summary Facilitates communication between the patient their primary care physician and specialists to improve clinical outcomes. Works collaboratively with the patient to foster selfmanagement and compliance to their clinical plan of care. Works collaboratively with physician hospital and community resources to support the patients clinical plan of care. Transition Navigators will work remote and travel to personal care homes. Requirements Education Bachelors Degree : in Nursing (Required) Masters Degree : in Nursing (Preferred) Experience 3 Years experience in disease or case management services with focus on telephonic management medical reconciliation and ambulatory care coordination (Required) General Experience with clinical hospitalbased information systems (Required) License/Certifications RNLIC PA Registered Nurse License (Required)
