ReMed a Collage Rehabilitation Partners is looking for a Full Time Nurse Practioner or Physcician Assistant to join our outstanding team in our residential programs located in Paoli Malvern and West Chester PA.
ReMed a Collage Rehabilitation Partner is a growing organization dedicated to excellence honesty and ethics in the treatment of our clients and each other. For more than 35 years we ve developed programs to meet the diverse needs of clients with moderate to severe traumatic and acquired brain injuries. We provide treatment for postconcussion syndrome stroke memory loss neurobehavioral issues and accident or sportsrelated head injuries.

Responsibilities

  • Provides medical oversight consultation and support to ensure that client medical issues and needs are addressed in a timely and comprehensive manner.
  • Completes routine and acute client assessments to maintain and/or restore overall health and wellness. This will be accomplished through routine visitation at each program and/or response to staff notification of a status change.
  • Provides interventions as needed; may also instruct staff on intervention processes and protocols.
  • Undertakes comprehensive history taking symptom assessment and examination upon request but not less than annually for all clients.
  • Utilizes diagnostics and consultation to determine etiology and plan of care.
  • Seeks out consultation and guidance from Medical Director and/or external physicians to provide care for clients.
  • Trains/educates family members funders and uses appropriate resources to develop and implement an individualized treatment plan for clients.
  • Coordinates nursing care activities in collaboration with other members of the interdisciplinary team to facilitate achievement of treatment plans and goals.
  • Is familiar with clients challenges prognosis functional status goals and needs for services and resources related to the admission/readmission; interfaces with treatment team to set up initial treatment plan and establish priority medical treatment goals.
  • Assists the RCM and clinical team in admission and discharge planning process recommending/ assessing and securing needed services modifying strategies and educating caregivers etc.; provides admission orders as needed and discharge prescriptions.
  • Participates in and/or provides necessary information for team meetings and interfaces with treatment team to establish rehabilitation goals and coordinates treatment.
  • Responsible for monitoring client s medical treatment plan and progress towards goals and revision of goals/treatment plan as necessary particularly in the area of medical stability.
  • Supports clinical team nurses case managers and therapists to ensure either consistent delivery of highquality care within the company s systems of care.
  • Takes responsibility for communicating all necessary medical/nursing information to appropriate team members physicians and other health care providers.
  • Responsible for ensuring consistent implementation of strategies through teaching team members eliciting feedback from team members reviewing medical documentation including trackers lab work etc.
  • Complies with professional standards of treatment licensure and state regulations and works within scope of practice.
  • Participates in scholarly activities including research and education. Participates in own professional development by maintaining required competencies identifying learning needs and seeking appropriate educational offerings.
  • Demonstrates ability to support clients in difficult situations (e.g. refusal of activities medications and strategies).
  • Works directly with the client and provides minimum to maximum assistance and/or supervision to promote the client s success in achieving identified treatment goals.
  • Provides direct clinical care as needed.
  • Serves as an educational resource to the client/family staff and provider team. Acts as a consultant to interdisciplinary health care team members. Responsible for participation in orientation and staff training regarding healthrelated issues.
  • Participates in the professional development of others. Acts as a preceptor and mentor to support the development of nurses nurse practitioners students and other health care providers.
  • Ensures completion of all necessary medical documentation including med progress notes changes to medication record and medication log forms etc. Responsible for recording the date and length of each client contact or service in the company s record keeping system.
  • Develops referral pathways with tertiary services to assure quality care/ interventions.


Requirements


  • Applicable state licensure as a registered nurse and masters or doctorate level degree from an NLNAC and/or CCNE accredited graduate school of nursing as an Advanced Practice Nurse in Adult or Gerontology are required.
  • At least five years of experience in an advanced practice role.
  • Experience in the rehabilitation of chronically or catastrophically ill or injured is preferred.
  • Demonstrated accountability and skills in analysis problem solving decision making related to medical nursing skills and time management is required.
  • The ability to communicate in writing and verbally with families clients staff and physicians; along with the ability to work successfully as an autonomous clinician and as part of the team is a must.
  • Maintenance of up to date skills knowledge and understanding and ongoing competencies in all aspects of the Advanced Practice Role as well as the ability to evaluate the safety and effectiveness of own clinical practice is expected.
  • Excellent planning organizational writing and time management skills required.
  • Ability to communicate with families professionals and coworkers is necessary.
  • Possession of a valid drivers license insurance and an acceptable driving record must be maintained at all times.
  • Competence in writing planning organization and documentation as well as time management skills are required.
