Multimedia Designer

SCG Healthcare Talent is currently recruiting "RN Case Manager" for a largest medical center in Texas.

• Coordinate and manage patient care from admission to discharge
  
• Develop and implement individualized patient care plans
  
• Collaborate with interdisciplinary teams to ensure continuity of care
  
• Monitor and evaluate patient progress, making adjustments to care plans as needed
  
• Communicate with patients, families, and health care providers to ensure coordinated care
  
• Facilitate patient transitions to appropriate levels of care
  
• Ensure appropriate utilization of resources and manage patient care within budget constraints
  
• Maintain accurate and up-to-date patient medical records
  

Requirements

• Active RN license
  
• Bachelors or Associates degree in Nursing
  
• 2+ years of experience in a clinical setting
  
• Knowledge of health care regulations and reimbursement systems
  
• Strong interpersonal and communication skills
  
• Ability to prioritize tasks and work independently
  
• Basic computer proficiency
  
• Ability to work in a fast-paced environment and handle multiple responsibilities.
