JOB TITLE: Family Medicine Physician DEPARTMENT: Medical LOCATION: Gypsum EFFECTIVE DATE: Fall 2023 FLSA: Exempt / Nonexempt REPORTS TO: CMOPOSITION SUMMARY Mountain Family Health Centers is a Federally Qualified Health Center located on the Western Slope of Colorado. We are seeking a part time to full time Family Medicine physician to care for some of our most complex patients in our integrated care clinics alongside our highly qualified medical providers and members of our dental and behavioral health teams. Although prenatal experience is preferred prenatal care is not a requirement of the position.ESSENTIAL DUTIES/RESPONSIBILITIES (Listed in descending order of importance) A. Comfortable practicing Family Medicine in a rural location1. Familiarity with managing complex patients without timely access to some specialties caring for the spectrum from newborns to geriatrics. 2. Prenatal care experience preferred with support from FPOB providers in our Avon location.3. Would be eligible for community privileges at two local hospitals (no admitting no call) B. Interest in serving underserved populations.1. Seeking mission driven individuals2. Comfort with low resource populations3. Bilingual in Spanish/English preferred C. Familiarity with Patient Centered Medical Home Model and Federally Qualified Health Centers1. Integrated care experience or strong interest in this model preferred as we practice with Dental and Behavioral Health providers in each clinic as well as Pediatric Psychiatric and Internal Medicine providers.2. JOB QUALIFICATIONSA. Education or Formal Training & Experience1. Has completed an ACGME accredited residency program in Family Medicine2. Board certified or board eligible in Family Medicine 3. Current and unrestricted license as MD/DO and DEA license. B. Requirements 1. Must pass a criminal background check.2. Annual Flu vaccine required. Employer will provide.3. Obtain PPD for tuberculosis (annually if provider). Employer will provide.4. Obtain vaccine records for MMR Varicella TDaP Hep B and COVID19. Employer will provide. C. Knowledge Skill & Ability: (include materials and equipment directly used)1. Work experience as a Facilities Coordinator or similar role.2. Strong knowledge of facilities management operations.3. Familiarity with office equipment and security systems.4. Hands on experience with facilities management software is a plus.5. Understanding of safety regulations in offices.6. Wellorganized. D. Physical Requirements and Workplace Environment: 1. Requires long periods of sitting telephone and computer work. 2. Requires ability to lift carry push and pull up to 25 pounds. 3. Requires ability to use routine office equipment such as computer and telephone.4. Requires sufficient near vision to be able to read documents and computer screen.5. Requires ability to comprehend both oral and written communications.6. OSHA Category II: Tasks in which the normal work routine involves no exposure to blood body fluids or tissue but exposure or potential exposure may be required as a condition of employment. 7. HIPAA Classification: unrestricted Access: A workforce member with unrestricted access will have full access to patient s protected health information including the patient s entire medical record for patient care purposes.
