Medical Case Manager

Role Description

This is a Medical Case Manager (RN OTR/L MSW) Fulltime Hybrid Role. The Medical Case Manager will be responsible for actively interacting with patients healthcare providers and payers to assess plan facilitate coordinate monitor evaluate and advocate for services and options to meet and individuals health and rehabilitation needs needs. The Medical Case Manager will also provide clinical knowledge and strategy support to team members conduct assessments develop care plans and coordinate resources and referrals.

Qualifications

• Bachelors degree in Nursing Occupational Therapy or Social Work.
• Current active and unrestricted RN OTR/L or MSW license in the state of Michigan.
• Minimum of 35 years of clinical experience in case management home health or clinical settings.
• Excellent problemsolving decisionmaking and interpersonal skills.
• Strong written and verbal communication skills.
• Ability to work collaboratively in a team environment.
• Proficiency in Microsoft Office Suite and Electronic Medical Record (EMR) systems.
• Ability to work independently and prioritize multiple assignments and projects.
• Strong computer with access to a reliable recent computer
• Excellent wifi access

Preferred qualifications:

• Case Management Certification (CCM) or related certification.
• Experience working with Medicaid and Medicare population.
• Bilingual language skills.

Physical requirements:

• Ability to sit or stand for extended periods of time while driving
• Ability to lift up to 15 pounds for computer files brief case
