A growing Healthcare Group is seeking a Licensed Practical Nurse (LPN) to work as a Care Coordinator for behavioral and mental health care of patients (BHI) or Chronic Care Management (CCM) while working out of an office located in Brooklyn NY.


Responsibilities:

  • Managing the behavioral and mental health care of patients.
  • Review patients medical records to ensure that their plan of care is being followed and that patients medical management needs are being met.
  • Working collaboratively with behavioral coordinators PMDs and psychiatrists to resolve any gaps inpatients treatment plans.
  • Your responsibilities will include chart audits and reviews medical charting maintaining a patient registry and attending care team meetings.

Requirements:


  • Valid LPN license in NY
  • Basic healthcare background experience
