Saint Alphonsus Medical Center in Ontario Oregon is seeking a locum OBGYN provider for coverage starting in March!
Dates Needed: March 15th at 0700 March 18th at 0700 March 22nd at 0700 March 23rd at 0700
Please make sure you have the following items readily available for your provider at the time of acceptance in order to expedite the credentialing process

CERTIFICATION REQUIREMENTS :
STATE LICENSE REQUIREMENTS :
Weekend Requirements : As part of schedule
On Call Requirements : 24 Hour Call
- Schedule: Must be on site no remote.
- 24 hour call coverage beeper call
- Locums will NOT be responsible for supervising other providers
- EMR: EPIC
- Level II Trauma
- Client will reimburse for travel
Please make sure you have the following items readily available for your provider at the time of acceptance in order to expedite the credentialing process
- MD diploma
- Internship/residency/fellowship certs
- All medical licenses
- DEA
- Board Certification
- Life support cards BLS
- Current and previous certificates of insurance
- Driver s license
- Professional color photo/headshot
- CMEs for the past 2 years
- Immunization records including TB Flu HepB Tdap Varicella and COVID
- References
- Case logs
Requirement description :
- Board Certified Required
- OR license Required
- DEA and CSR Required
- BLS Required
- Local candidates highly preferred
- Must meet perm rate cap of BR5k
- *Must submit proof of COVID vaccine in order to work at this facility Required at time of submission**
