Saint Alphonsus is in need of a Trauma Surgery APP for 7 on/7 off starting in June!
Details:
Please make sure you have the following items readily available for your provider at the time of acceptance in order to expedite the credentialing process
Requirement description :
CERTIFICATION REQUIREMENTS :
STATE LICENSE REQUIREMENTS :
Weekend Requirements : TBD
On Call Requirements : No Call
Details:
- Shifts vary will accept either day or nights 7a7p 7p7a. Must have 7 on/7off availability.
- Not doing daily rounding on patients just responding to traumas
- EMR: Epic
Please make sure you have the following items readily available for your provider at the time of acceptance in order to expedite the credentialing process
- MD diploma
- Internship/residency/fellowship certs
- All medical licenses
- DEA
- Board Certification
- Life support cards BLS
- Current and previous certificates of insurance
- Driver s license
- Professional color photo/headshot
- CMEs for the past 2 years
- Immunization records including TB Flu HepB Tdap Varicella and COVID
- References
- Case logs
Requirement description :
- NCCPA/AANP Required
- Active ID license or IMLC Highly Preferred but will accept licensing candidates.
- ACLS Required
- Local candidates no flights HIGHLY PREFERRED
- Must have previous Trauma experience to be considered.
- Must have 7 on/7off availability to be considered!
CERTIFICATION REQUIREMENTS :
- ACLS (AHA)
- Board Certified
STATE LICENSE REQUIREMENTS :
- Idaho
