UCHealths Little Snake River Clinic is looking for a locum Primary Care APP to help fill their need in Baggs Wy.
Details:
Travel Requirements:
Rates to Submit:
- Malpractice Must be included in the rates.
- Orientation per hour of orientation or training
- Hourly Per hour Must disclose where hourly ends.
- Overtime Per hour Must disclose when OT starts.
- All corresponding holiday rates.
- Conversion Fee 25K cap set out by UCHealth.
Requirement description : ALL OF THE FOLLOWING MUST BE NOTED AT THE TIME OF NAME CLEAR IN THE ADDITIONAL SUBMISSION DETAILS TO AVOID BEING REDIRECTED!!
- AANP/NCCPA Certified REQUIRED
- Active WY License REQUIRED
- Active ACLS (AHA) BLS (AHA) DEA REQUIRED
- Must state providers availability (# of shifts per month) at name clear REQUIRED
- Must disclose the # of years of Epic Experience and the Last MM/YYYY provider used EPIC REQUIRED
- Clean Malpractice/Background REQUIRED
- COVID VACCINATED REQUIRED
CERTIFICATION REQUIREMENTS :
- ACLS (AHA)
- BLS (AHA)
- Board Certified
- DEA
STATE LICENSE REQUIREMENTS :
- Wyoming
