Tasks

• Provide health coaching services to residents to improve their physical and mental wellbeing
• Develop individualized health plans based on residents needs and goals
• Educate residents on healthy lifestyle choices including nutrition and exercise
• Collaborate with healthcare professionals to ensure residents receive comprehensive care
• Document and track residents progress and adjust health plans as needed

Requirements

• Bachelors degree in Health Education Nursing Public Health or related field
• Certification as a health coach or wellness coach
• Strong communication skills and ability to build rapport with clients
• Knowledge of health and wellness topics including nutrition exercise and stress management
• Experience working with diverse populations and promoting behavior change
