Key Responsibilities:

• Document assessment and interventions according to policy and practice standards.
• Conduct bedside patientcentered rounds using the SBAR tool for effective communication.
• Mentor and precept new staff and students.
• Provide crisis intervention for our unique population.
• Complete postEmergency Department (ED)/Urgent Care and postdischarge phone calls within 2 business days.
• Actively manage a highrisk registry of patients with specific health concerns.
• Collaborate with the Teamlet and expanded Team to develop patientdriven care plans.
• Provide patient and family health education focusing on selfmanagement prevention and wellness.
• Manage triage telephone triage and scheduled and unscheduled patient visits.
• Execute nursedriven clinics to support positive health outcomes.
• Promote costeffective use of resources and adopt effective nonfacetoface approaches including TeleHealth.
• Keep providers and case managers aware of patient progress issues and problems.
• Screen patients for social service home care and community care needs; coordinate referrals as appropriate.
• Use xray machine including operative Carm xray machine and wear lead protective apron.

Preferred Experience:

• Experience with operative Carm xray machine.
• Strong communication and assessment skills.
• Triage experience with veterans dealing with pain issues.
• Work experience in a spinal procedure suite.
