Complex Care Nurse

Seasoned Recruitment is currently seeking a dedicated and compassionate Complex Care Nurse to join our team. As a Complex Care Nurse you will be responsible for providing highquality nursing care to patients with complex medical conditions. You will work in collaboration with physicians other healthcare professionals and the patients support system to develop and implement individualized care plans that promote patient wellbeing and improve health outcomes.

RESPONSIBILITIES:

• Conduct comprehensive assessments of patients medical conditions including physical psychological and psychosocial factors
• Develop and implement individualized care plans based on patients unique needs and goals
• Administer medications and treatments as prescribed by physicians
• Monitor and assess patients response to interventions adjusting care plans as necessary
• Provide education and support to patients and their families to promote selfmanagement and optimal health
• Collaborate with interdisciplinary healthcare team members to ensure coordinated and integrated care
• Document all patient care activities accurately and in a timely manner

REQUIREMENTS:

• Graduate of an accredited nursing program
• Current nursing license in the state of practice
• Strong assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work effectively in a multidisciplinary team
• Proficiency in electronic health record systems
