Elite Staffing Network is seeking a Care Manager RN for Monogram Health in Saint Paul MN.
qualifications
required
- Valid Registered Nurse (RN) license/certification in Minnesota (MN)
- 2 years experience in any of the following:
- Dialysis Care
- Home Health Care
- Hospice
- Case Management (CM)
- Are you seeking a fulltime position (yes)
- Are you comfortable traveling to patients homes (yes)
- The work schedule is MF from 8 AM 5 PM. Are you available and comfortable with this schedule (yes)
- You will be travelling within a onehour travel radius from the main location within which you will be visiting patients inhome. The job is 34 days on the road visiting patients and 12 days telehealth. Are you comfortable with this and willing to put in the road time (yes)
preferred
- Are you bilingual English/Spanish (yes)
