RN Case Manager with experience as follows:Must have strong Acute Care Discharge Planning experience.Conducting patient initial assessmentsAcute Care hospital experienceEPIC EMR documentation experienceActive CA RN LicenseMonday through Friday 0800 to 1630 / alternating weekends.Eight hour shifts only.40 hours guaranteed.
