Job Summary
The Care Partner Support at Home works closely with older people to coordinate their government funded aged care services helping them live safely and independently in their own homes. Using a person-centred approach the role supports consumers to make informed decisions access appropriate services equipment and home modifications and adapt their care as needs change. The Care Partner also coordinates service providers and manages allocated funding to ensure supports align with each individuals goals preferences and wellbeing with guidance from qualified health professionals.
This is a permanent permanent opportunity working 3 days per week (flexibility to choose days).

Are you a compassionate and organised professional looking to make a real difference in the lives of older people We are seeking a motivated Care Partner Support at Home who enjoys building relationships and coordinating services to support individuals to live independently at home.
Key skills and experience include:
- Minimum 2 years experience in aged care care support or transferable role
- Experience building relationships with consumers families and service providers
- Understanding of My Aged Care system portal and referral pathways
- Strong communication interpersonal and stakeholder engagement skills
- Ability to negotiate manage conflict and problem solve effectively
- Ability to work independently and within a team with sound judgement
- Working knowledge of wellbeing and reablement principles
